  3. Signing up a new group and enrolling members is easy!
    1. Group Application" to provide us with basic information we need to properly service a new case. It will also allow the group to select the products that will be offered AND to choose billing and service options.
    2. "Member Enrollment Form" for anyone interested in joining the program! Only basic information is required and acceptance is guaranteed!
